Youth Advocacy and Parenting Support Program

Working with participants of Faith Works Programs to reduce adverse childhood experiences and toxic stress.

Our onsite Youth Advocate provides one on one meetings, sibling sessions and group activities with the youth who live onsite. Additionally, our dedicated youth advocate provides parental support, parenting education and assistance with referrals and events. Some content focused on during youth services is emotional regulation, anger management, milestones, education support, life skills, social skills, and healthy relationship building.


Enhanced Care Management and Community Supports for homeless and at-risk

FaithWorks has partnered with Partnership Health Plan to provide California Advancing and Innovating Medi-Cal (CalAIM) services. This allows us to provide unique services to Shasta County individuals (adults and youth) experiencing homelessness or who are at risk of homelessness.

Enhanced Care Management or ECM Services - Case managers provide holistic case management to promote health and overall wellbeing by identifying barriers and making a plan to address the barriers.

Community Supports- Case managers work with clients to navigate housing needs and/or help individuals sustain housing.

To be eligible, you must be homeless or housing unstable in Shasta County and have active Partnership. We cannot take clients who are already established with another ECM or CS provider. For more information call our main office at 530-242-1121 or contact Partnership HealthPlan of California.
